Here is a printable vintage Bill of Lading tag. The first side includes lines to write the bill of lading number, the nearest rail station, total weight, number of pieces and a description. There are small blocks to include any relevant VIA and ZONE information.
The second side of the bill of lading tag indicates the items are being shipped from: S. O. Tadcen, Treasure Island, San Francisco, Calif. There are lines to include sender’s last, first and middle names, the rate, service number and date. There are also lines to include the receiver’s name for the items being shipped, as well as their city, county, and state.

I’m not sure of the exact time frame; however, I have two of these original tags that were filled in (handwritten). The dates on both were in the 1940s. Hope that helps.
